How CO2 laser resurfacing works
Of all the treatments I perform, CO2 laser resurfacing is the one I get asked about most — and the one most often misunderstood. People tend to picture one thing: a week indoors with a red, peeling face. That's one version of it. It isn't the only one.
The basic idea: controlled injury, controlled repair
A CO2 laser produces light at a wavelength that water absorbs almost completely. Your skin is mostly water, so that energy turns to heat the instant it lands and removes tissue in precise microscopic columns — what the research literature calls microthermal treatment zones.
The important word is fractional. Instead of removing the entire surface of your skin, we treat a fraction of it and leave healthy islands in between. Those untreated islands are what heal you. They supply the intact cells that resurface the treated columns, which is why modern fractional lasers recover in days rather than the weeks required by the fully ablative resurfacing of the 1990s.
Beneath the surface, that controlled heat starts a real wound-healing response. Fibroblasts — the cells that manufacture collagen — get activated, and they keep working for months. That's the part patients tend to underestimate. The peeling is over in a week. The collagen remodeling is what you're actually paying for, and it continues long after your skin looks normal.
Why CO2, specifically
There are gentler devices, and I use them. But CO2 remains the reference standard for resurfacing for one reason: it does two jobs at once.
It removes damaged surface tissue and delivers heat into the deeper dermis in the same pass. Most other technologies do one or the other. That dual action is why a single well-executed CO2 treatment can improve texture, tone, and firmness together, rather than requiring three separate devices to chase three separate problems.
What it treats well
In my practice, CO2 resurfacing does its best work on:
Texture and pore size — roughness, orange-peel skin, enlarged pores
Fine lines — especially the etched lines around the mouth and eyes that don't respond to neuromodulator
Sun damage — the cumulative brown, mottled, leathery quality of decades of UV exposure (which, in Marin, most of us have)
Acne scarring — atrophic and rolling scars, where the collagen rebuilding genuinely lifts depressed tissue
Overall dullness and laxity — early crepiness, particularly on the neck and décolleté
What it isn't for
I'd rather tell you this in a blog post than have you find out in a treatment room.
CO2 will not replace volume. If your concern is hollowing in the cheeks or temples, that's structural loss, and resurfacing the surface won't address it. It also won't lift significant sagging — jowls and heavy neck laxity are a surgical conversation, and I'll tell you so directly. Deep dynamic wrinkles caused by muscle movement respond better to neuromodulator than to a laser.
There are also timing considerations. We don't treat over active breakouts or an active cold sore, during pregnancy or breastfeeding, or too soon after isotretinoin. Melasma is its own category — it can respond to CO2, but it can also flare, so it requires a cautious, conservative plan rather than an aggressive one. And richer skin tones carry a higher risk of post-inflammatory pigment change, which means gentler settings and a longer runway. None of these are automatic disqualifications. They're reasons the consultation matters.
Downtime is a dial, not a fixed price
Here's what most people don't realize: on the DEKA SmartXide Tetra Pro we use, I control the shape of the energy pulse, not just its strength.
On CoolPeel settings, very high power is delivered in an extremely short burst — the energy resurfaces the superficial layer and exits before meaningful heat can spread into surrounding tissue. You're typically pink and a little sandpapery for one to three days.
On deeper DEKA settings, we intentionally leave heat behind to reach the dermis, which is what acne scarring and deeper sun damage actually require. Expect five to seven days of visible peeling, with residual pinkness fading over a few weeks. The Cleveland Clinic puts full CO2 recovery at up to two weeks.
Neither is better. They're different tools for different goals.
How long results last
Honestly? Longer than most people expect, and not forever.
The collagen you build is real structural tissue, and it doesn't disappear when the treatment wears off — there's no "wearing off" in the way there is with a neuromodulator. Patients who have a deeper treatment often hold meaningful improvement for several years. What varies is everything happening alongside it: you continue to age, and sun exposure continues to undo the work. The single biggest predictor of how long your results last is whether you wear sunscreen daily. That isn't a sales line; it's the actual variable.
For most people, a sensible rhythm looks like this: a deeper session to address a specific concern, then two to three light CoolPeels a year as maintenance — steady collagen stimulation with almost no disruption to your calendar.
